Short Term Case Manager Charis House

Position Description

To provide appropriate case management for assigned residents of the short term program.

Key Tasks and Responsibilities

  • Reflect personal integrity in attitude and behavior toward clients, volunteers and other staff.
  • Assure resident needs are addressed (i.e. agency referrals, letters of residency, clothing, transportation, etc.)
  • Participate in training to improve skills and increase competency.
  • Communicate/coordinate with agencies (Park Center, LSSI, Safe Harbor, etc.) involved with resident care and attend team meetings when applicable.
  • Facilitate SMART goal-setting with all assigned residents and document progress.
  • Facilitate the completion of all client-related paperwork (Medicaid forms, SSI applications, rental/job applications, etc.) as needed.
  • Obtain signed releases of information from each resident for all agencies as needed.
  • Facilitate the transition for clients choosing to exit.
  • Facilitate groups as assigned.
  • Facilitate the understanding of guidelines by clients and address when not being followed.
  • Develop, implement, and facilitate Vocational training and groups
  • Communicate regularly with appropriate staff through staff log and email when face to face is not possible.
  • Perform brief assessments as needed.
  • Work in conjunction with the Vocational Team to help residents meet their vocational goals.
  • Complete all client-related documentation in a timely manner.
  • Administer substance abuse tests.
  • Provide transportation when needed for residents.
  • Perform on-call duties.
  • Perform other duties as assigned by the Clinical Director or appropriate Team Leader.
  • Review and process each of the 12-steps with each assigned Long Term resident.
  • Provide information on assigned residents for staff meetings.
  • Demonstrate professional, ethical behavior including boundaries with residents.
  • Participate in training to improve skills and increase competency.

Education and Training

College degree or equivalent experience in related field.

Knowledge and Experience

  • Computer knowledge

  • Ability to relate to all persons with dignity and respect.
  • Strong written and verbal communication skills.
  • High level of self-motivation.
  • Flexibility in work hours when needed to assure proper coverage.

Physical Demands

While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close vision.
